4. Billing Fees

A fee of 1,000 yen will be charged for each application only for requests for notification of the purpose of use or disclosure of personal information or records of provision to a third party. Please enclose 1,000 yen worth of postage stamps with your application documents.
If the fee is insufficient or not enclosed, we will contact you to inform you, but if payment is not received within the specified period, we will treat the request for disclosure as if it had not been made.

5. How to respond to requests for disclosure, etc.

Regarding notifications and disclosures based on your requests, we will respond to you in writing to your address or by providing electromagnetic records (messages in the system, emails) in accordance with the method specified on the invoice.
